Department of Public Instruction - Traumatic Brain Injury
As a Caregiver and as an Athlete it is important to recognize the signs, symptoms, and behaviors of concussions. By signing this form you are stating that you understand the importance of recognizing and responding to the signs, symptoms, and behaviors of a concussion or head injury.
Family Agreement:
I have read the Traumatic Brain Injury Information and understand what a concussion is and how it may be caused. I also understand the common signs, symptoms, and behaviors. I agree that my child must be removed from practice/play if a concussion is suspected. I understand that it is my responsibility to seek medical treatment if a suspected concussion is reported to me. I understand that my child cannot return to practice/play until providing written clearance from an appropriate health care provider to his/her coach. I understand the possible consequences of my swimmer returning to practice/play too soon.
- A total of 8 hours of shifts per swimmer is required at home meets, with a maximum of 16 hours per family. Home meets are on 12/14/24 and 1/18/25.
- 4 hours of volunteer work per swimmer at a fundraiser (other than a home meet), with a maximum of 8 hours per family. Fundraiser examples include Wolfman, Culver’s night, brat fry, optimist cheese curds, parades, etc.
- Must sell one raffle ticket booklet per swimmer.
Opt-Out Option:
If you prefer to opt out of fulfilling the volunteer requirements for the season, you may do so by paying a fee of $400 per athlete at the time of registration. To opt-out, simply click "Yes, I would like to opt-out." Please note that if you choose to opt out, or if you fail to fill a volunteer position, a fee of $100 per 4 hour shift/raffle booklet per swimmer will be incurred.
Important Reminder:
All volunteer hours must be completed, or the applicable fees must be paid, before you can register for the following season.

Swimmer Pledge
Antigo Penguin Club members are expected, as a condition of their membership, to affirm and abide by a Code of Conduct and responsibility that engenders fair play, respect for elders and peers, and self-discipline, as well as camaraderie, team spirit and safety.
As a member of the Antigo Penguins Swim Club, acknowledge that following certain basic rules of conduct is important to my development as a swimmer, my relationship with my family, coaches, and teammates, the community reputation of my team, and the preservation of the many relationships that contribute to the success of Penguins Swimming. I realize that not acting in a proper way while engaged in Club activities is harmful to myself and to my team, and prevents me from becoming the best swimmer and person I can be. I therefore pledge and commit to observe the following basic rules of conduct while involved in Antigo Swim Club-related activities:
I pledge to treat with courtesy and respect my fellow swimmers, coaches, team parents, School District employees, meet officials, parent volunteers and swimmers, coaches and families of other swim teams.
I pledge that my behavior at practice and at meets will be safe and appropriate.
I pledge not to interfere with the workouts of other swimmers.
I pledge to obey all lawful directions of adults with appropriate authority promptly and respectfully.
I pledge not to threaten Club members, adults, or swimmers.
I pledge not to fight with hit or trip, bully or engage in other harmful acts toward other swimmers and adults.
I pledge to use appropriate language and to refrain from swearing, name calling or racial/ethnic/gender or homophobic slurs.
I pledge that I will not vandalize property, steal, and possess weapons or other dangerous items.
I pledge not to use or possess alcohol, drugs, or other controlled substances.
I understand that violating these Penguin Swimming Rules of Responsibility could lead to and may include requiring a conference with caregivers, loss of practice and/or meet privileges and suspension or removal from the team.
Discipline Guidelines
For verbal abuse, which consists of any verbal assault-however mild-on any person, be it fellow swimmer, coach, parent, or anyone acting as deck helper; name calling, insults, or any other speech that is considered offensive within reason, be it in the pool, on deck or in the locker room:
For physical abuse, which shall consist of any aggressive contact with mean intention towards another swimmer-be it grabbing or pulling on any part of the body, splashing water in another swimmer's face to cause harm or irritation, intentional kicking to cause harm, intentional spitting to cause harm or irritation, swimming intentionally so as to collide head on with another swimmer or any other behavior that may be constructed as physically aggressive, cruel, or detrimental to the team:
If the assault is especially heinous or goes beyond physical assault, per the offended swimmers' family or a coach, the matter will be considered too serious for the swim club and will be referred to the Antigo Police Department.
Antigo High School Athletes will follow the Antigo High School Athletic Code. Any discipline due to a combination of offenses will be taken consecutively. Any grievances must be addressed to the Antigo Swim Club Board President.
Family’s Code of Ethics
Antigo Penguins Swim Club is committed to providing our athletes with an environment that will enable them to achieve their potential as well as an environment in which all individuals (athletes, coaches, staff and families) are treated with respect. An encouraging family goes a long way in the development and achievement of a swimmer and affects the environment that they train/compete in. We like to promote a supportive atmosphere for all of our athletes and families. Support your swimmers’ sport of choice!
● I will conduct myself at all times in a manner consistent with the values of the Antigo Penguins Swim Club, which include fairness, integrity and respect. This pertains to interaction with all athletes, other families, officials, staff and coaches.
● I will refrain from comments or behavior that is disrespectful, offensive, racist, abusive or sexist.
● I will not engage in destructive gossip regarding coaches, staff, families, athletes or officials.
● If I have questions or concerns with respect to my child’s swim program, I should contact my child’s coach directly. Any further concerns will be directed to the Head Coach. If my concern is still not resolved, I will contact someone on the Swim Board.
● I will not approach coaches or officials on the swim deck during swim meets or practices. Instead, I will arrange to meet with coaches before or after the sessions.
● I will ensure that my child arrives at practice on time.
● I will encourage a healthy lifestyle including nutritious foods and adequate sleep.
● In support of team spirit and unity, I will ensure that my swimmer will have the proper equipment and team attire as per the equipment list for their swim group, as distributed at registration.
● I will be supportive of my child and the team at all times, regardless of performance level.
● I understand that volunteer organizations such as Antigo Penguins Swim Club depend on the energy and goodwill of families to fulfill numerous, essential roles. When I have an issue with meet coordinators or Board members, I will respect that these are all volunteers working on my child’s behalf. If I feel the need to take issue with any member of our ‘team’, I will do so privately and directly with the people involved. If resolution is not achieved, I will contact the Swim Club Board
● I will ensure that my swimmer understands that internet web sites, such as, but not limited to, Facebook, on which people socialize and exchange information, shall not be used to post remarks or pictures that can be considered inflammatory, degrading or in poor taste toward any other athletes (Penguins or otherwise), coach or volunteer.
● I will support my swimmer and the Club by volunteering for all home meets as well as to assist in social events and fundraising activities. Total volunteer hours required this year is 16 hours per swimmer. A productive and safe atmosphere promoting camaraderie and family values is accomplished through the collaborative efforts of our families, athletes, coaches, and staff. With these values in mind, I understand that failure to comply with these Codes may result in appropriate action as determined by the Board of Directors, up to and including termination of membership.

Photo and Video Consent Statement
I, [Parent/Guardian], give permission for Antigo Swim Club to use photos and videos of my child, for promotional and informational purposes on the club's website, social media platforms, and other marketing materials. I understand that these photos and videos may be used to showcase the achievements and activities of the Antigo Swim Club and its members.
I acknowledge that the photos and videos will be used in a respectful and appropriate manner and will not be shared with any third parties without my consent.
I agree that this consent is valid for the duration of my child's membership with Antigo Swim Club, unless otherwise revoked in writing. I also acknowledge that I have the right to request the removal of any specific photos or videos featuring my child at any time.
Equipment Deposit:
- A deposit of $50 will be required at registration for an equipment bag, which will include a pair of fins and a snorkel.
- This deposit will be refunded at the end of the season upon the return of the items in good condition.
Important Notes:
- If your child already owns a pair of fins and a coach approved snorkel, you do not need to pay the deposit.
- Additionally, swimmers have the option to use a locker at the pool to store their equipment. If you choose this option, please ensure that your swimmer brings a lock to secure their belongings.
If you click "yes," a $50 deposit fee for equipment will be charged. If you choose not to click "yes," your child will be required to provide their own coach-approved snorkel and fins for all practices.
